Please note that intake & waiver forms are required for EVERY person receiving services to ensure compliance with our safety protocols and to maintain the highest standards of service delivery.

Journey Within Wellness Center kindly requests that all guests complete a waiver form for each service that they intend to receive. (If you are receiving multiple services, please complete a waiver for each service.)
